What to Do When You Lose Medi-Cal Coverage in California

A Medi-Cal termination notice does not necessarily mean you must go without health insurance. The correct next step depends on why coverage ended: missing renewal information, a change in eligibility, or an administrative mistake. Acting quickly can help prevent or shorten a coverage gap.

Start with the date and reason on your Notice of Action. Keep the notice, renewal documents and proof of any information you submit.

First, find out why your Medi-Cal ended

  • Missing renewal information: The county may not have received a completed renewal form or requested documents.

  • Income or household change: Your income, household size, tax status or other eligibility information may have changed.

  • Administrative mistake: Information may be missing, outdated or recorded incorrectly.

  • A program-rule change: Certain eligibility, asset or work-related rules may apply depending on your age and Medi-Cal category.

If you missed your Medi-Cal renewal

Contact your county Medi-Cal office as soon as possible. California's Department of Health Care Services says that if you provide the missing renewal information within 90 days and remain eligible, the county can reinstate Medi-Cal without requiring a new application and without a coverage gap.

If more than 90 days have passed, contact the county and submit a new application. Do not assume the old case will reopen automatically.

If you no longer qualify for Medi-Cal

Losing Medi-Cal is a qualifying event for a Covered California special enrollment period. Apply promptly, compare available plans and choose coverage before the deadline shown in your eligibility or transition notice.

A Covered California application can also determine whether anyone in the household still qualifies for Medi-Cal or whether the household qualifies for premium tax credits and Enhanced Silver savings.

How to avoid a gap in health coverage

  1. Read the Notice of Action and confirm the exact Medi-Cal end date.

  2. Contact the county immediately if the decision appears incorrect or documents were missing.

  3. Update or complete the Covered California application using current income and household information.

  4. Select a plan promptly; completing an application alone may not finish enrollment.

  5. Confirm the new plan's effective date and make the first premium payment by the insurer's deadline.

  6. Before enrolling, verify doctors, hospitals, prescriptions, deductible and maximum out-of-pocket costs.

Documents to gather

  • The Medi-Cal Notice of Action or termination notice

  • Recent pay stubs or self-employment income records

  • Current household and tax-filing information

  • Immigration documents when applicable

  • Employer-coverage information, including the employee premium

  • Names of doctors, hospitals and prescription medications

If Medi-Cal ended by mistake

Contact the county office listed on the notice and explain why the decision appears incorrect. Ask what information is needed and how to submit it. The Notice of Action also explains appeal or State Fair Hearing rights and applicable deadlines.

Keep confirmation numbers, upload receipts, fax confirmations and copies of documents. If medical care is urgent, tell the county or enrollment assister immediately.

Special situations to review

  • Pregnancy, disability, Medicare eligibility or long-term-care needs may affect the Medi-Cal pathway.

  • Children may qualify for Medi-Cal even when adults in the household do not.

  • A move to another California county usually requires an inter-county transfer rather than ending coverage.

  • Beginning January 1, 2027, some adults may receive notices about new work, school, training or volunteer requirements. Respond promptly if Medi-Cal says the rule applies to you.

Understanding possible Covered California savings

Income benchmarks can help with planning. Review the site's 2027 Covered California income limits and subsidy guide for household examples and Enhanced Silver information.

The final premium and eligibility result depend on the complete application, including income, household size, ages, ZIP code and access to other qualifying coverage.

Frequently asked questions

Can I enroll in Covered California after losing Medi-Cal?

Yes. Losing Medi-Cal is generally a qualifying event for special enrollment. Apply promptly and follow the deadline and effective-date instructions provided with your transition or eligibility notice.

Can Medi-Cal be restored after a missed renewal?

Potentially. If you submit missing information within 90 days and are still eligible, the county may reinstate coverage without requiring a new application.

Does submitting a Covered California application activate a plan?

Not always. You may still need to select a plan, confirm enrollment and pay the first premium by the carrier's deadline.

What if I disagree with the Medi-Cal decision?

Contact the county and review the appeal or State Fair Hearing instructions on your Notice of Action. Deadlines apply, so act promptly.
